Job summary

Memorial Healthcare System seeks a qualified professional to lead Diabetes Self-Management Education and Support Training (DSMEST). You will provide patient education, develop care plans, and collaborate with care teams to improve outcomes.

Two years of DSMEST experience are required, with strong communication skills and the ability to work under pressure with minimal supervision. RN, RD, or related credentials are preferred.

Required Work Experience

Two (2) years' relevant work experience pertinent to Diabetes Self- Management Education and Support Training (DSMEST).Experience with program planning and evaluation (i.e., pre- and post-test, repeat demonstration, clinical measures, evaluating trends, and communicating plan of care).
