Qualifications

Responsibilities

  • Conduct outreach calls and risk assessments for Depression Screening Follow-Ups and SDOH measures.
  • Provide individual and group psychotherapy coverage as needed.
  • Complete intake assessments (PHQ-9, GAD-7, ACEs) and biopsychosocials.
  • Develop SMART treatment plans and monitor progress toward goals.
  • Coordinate care and linkages for patients and families.
  • Maintain timely charting within agency policy (72 hours).
  • Collaborate with multidisciplinary team to ensure holistic care.

Job Title: Special Projects

Salary: $44+ hourly DOE

Location: Tustin , CA

Positions: (1) Full-time

Position Purpose:

Provides support for departmental special projects, including outreach, follow-up, and care coordination activities related to Social Determinants of Health (SDOH) measures, depression screenings and other UDS or quality measures, as assigned. Delivers administrative support and operational coverage for mental health services, including individual therapy, group therapy, and crisis intervention programs, ensuring continuity of care.

Core Duties and responsibilities include, but are not limited to:
  • Conduct outreach calls, perform risk assessments, develop care plans, and facilitate linkages for Depression Screening Follow-Ups, SDOH interventions, and other UDS or quality measures
  • Accurately maintain and update spreadsheets to track progress and outcomes for special projects
  • Provide individual and group (offsite and onsite) psychotherapy coverage, as needed, utilizing evidence-based practices
  • Establish rapport with patients, and establish trust and safety in the therapeutic space
  • Complete intake assessments (including PHQ-9, GAD-7, ACEs, etc.) and complete biopsychosocial to gather necessary information for patient care
  • Complete treatment plan collaboratively with patients using SMART goals and implement appropriate and relevant treatment interventions
  • Conduct ongoing assessment of patient progress in meeting treatment goals
  • Facilitate discharge planning and provide necessary linkages for further support, as needed
  • Respond to crisis situations and intervene therapeutically as necessary, including assessing for safety of patient, suicidality, homicidality, and self-harming behaviorsAble to clinically conceptualize patients’ behaviors based on needs and strengths
  • Able to incorporate patient choice into treatment model
  • Refer patients to outside resources and support as appropriate and needed
  • Follow all safety protocols and maintain patient confidentiality
  • Collaborate within multidisciplinary team to ensure whole-person treatment is implemented
  • Maintain current and acceptable standards of charting and documentation within agency policy of 72 hours
  • Exercise independent judgement in clinical matters within scope of practice
  • Actively participate in clinical supervision and consultation, until full licensure is obtained

Work Schedule:
  • Monday - Friday 9 AM - 6 PM.
  • This position is fully on site, you are expected to report to the Tustin, CA location daily.
  • Work schedules are subject to change based on organizational, staffing, community, and patient needs. As such, FTOC may need to modify work schedules to meet such needs.
