We are seeking a stellar part-time Special Operations Case Manager who will provide a full range of professional health nursing principles, practices, and procedures in clinical settings. The role involves analyzing problems related to providing appropriate, cost-effective care to Department of Defense (DOD) beneficiaries. Essential Duties & Responsibilities:

  1. Collect, organize, record, and communicate data relevant to primary health assessments, including detailed medical histories, to develop timely treatment plans outlining the care process for selected case-managed patients or populations.
  2. Assess patients via telephone using established protocols to provide appropriate and cost-effective care.
  3. Establish priorities for patient care, monitor, and evaluate progress toward goals to ensure coordinated, efficient, and effective healthcare delivery.
  4. Oversee discharge planning activities to ensure optimal timing and sequencing of patient care.
  5. Assist healthcare finders in identifying patient needs for referrals to appropriate healthcare providers or facilities.
  6. Negotiate, draft, finalize, and administer product/service agreements to ensure mutual compliance with care goals.
  7. Utilize pre-established utilization review criteria to recognize and report quality and risk management issues, aiming to improve practice and ensure compliance.
  8. Apply VA/DOD Clinical Practice Guidelines to ensure evidence-based, standardized care for military beneficiaries.

Required Education/Experience/Skills/Training:

  1. U.S. Citizenship is required.
  2. Ability to obtain and maintain a Secret or Top Secret Clearance.
  3. Demonstrated skill in understanding diagnoses to help patients optimize their function and self-care.
  4. Current, unrestricted licensure as a Registered Nurse or Licensed Clinical Social Worker in the U.S.
  5. Minimum of two (2) years of nursing or case management experience, with certification as a case manager by a recognized organization (e.g., CCM, ANCC). Certification must be obtained within the first year if not already held.

Additional Skills and Knowledge:

  • Knowledge of patient education principles and management of complex medical, psychosocial, and financial issues.
  • Ability to seek and incorporate feedback to improve practice and expand knowledge.
  • Effective consultation and negotiation skills with healthcare providers and outside agencies.
  • Possibility to receive and maintain clinical privileges and credentialing at local MTF.
  • Fluent communication skills in English, both oral and written.
  • Team coordination, facilitation, and communication skills to support goal achievement and system change.
  • Knowledge of applicable laws, regulations, and ethical standards.
  • Current certification in Basic Cardiac Life Support (BCLS) by the American Heart Association, with proof of certification required.

Standard Company Requirements:

  • Compliance with safety, health, environmental, and quality assurance policies.
  • Regular attendance and completion of required training.
  • Perform other duties as assigned.

  • Previous military background or experience is preferred.
