We seek a Special Operations Case Manager to provide comprehensive nursing principles and practices in clinical settings, analyzing problems related to cost-effective care for Department of Defense (DOD) beneficiaries.

Essential Duties & Responsibilities
  1. Gather and organize data for health assessments to develop timely treatment plans.
  2. Assess patients via telephone using established protocols to ensure appropriate care.
  3. Prioritize patient care, monitor progress, and evaluate outcomes for effective healthcare delivery.
  4. Manage discharge planning to optimize timing and sequencing of care.
  5. Assist healthcare finders in identifying patient referral needs.
  6. Negotiate and manage care-related agreements ensuring compliance.
  7. Utilize utilization review criteria to identify quality and risk management issues.
  8. Apply VA/DOD Clinical Practice Guidelines to ensure standardized, evidence-based care.
  9. Manage healthcare resources efficiently considering financial and population health aspects.
  10. Use research tools to evaluate practices and outcomes.
  11. Manage information systems to improve practice and communicate outcomes.
  12. Design and present data analyses for educational and briefing purposes.
  13. Understand pharmaceuticals to monitor effects and manage side effects.
Minimum Requirements
  • U.S. Citizenship is required.
  • Ability to obtain a Secret or Top Secret Clearance.
  • Active, unrestricted RN or Licensed Clinical Social Worker license in the U.S.
  • At least two years of nursing or case management experience; certification as a case manager within the first year preferred.
  • Knowledge of patient education and managing complex medical, psychosocial, and financial issues.
  • Strong communication, negotiation, and team coordination skills.
  • Maintain clinical privileges and credentialing in local MTFs.
  • Proficiency in English communication, both oral and written.
  • Current Basic Cardiac Life Support (BCLS) certification from the American Heart Association.
Standard Company Requirements
  • Compliance with safety, health, environmental, and quality policies.
  • Regular attendance and completion of required training.
  • Perform additional duties as assigned.
Desired Qualifications
  • Military background or experience is preferred.
