Job summary

Family Pillars Homecare, LLC is seeking dedicated RNs and LPNs to provide skilled home health care in Florida on a per diem basis. The role includes intermittent visits, private duty shifts, and care plan coordination. Licensed Florida RNs or LPNs who are reliable are encouraged to connect.

Qualifications

• Active Florida RN or LPN license (Compact License accepted).
• Current CPR certification (BLS).
• Level 2 background screening (AHCA-compliant).
• TB screening and physical within past 6 months.
• Experience in Medicare-certified home health preferred.
• Familiarity with EMRs (Axxess) and knowledge of PDGM & OASIS for RNs.

Responsibilities

• Perform skilled nursing visits including wound care and IV administration.
• Medication management and patient assessments.
• Document patient condition and progress; update care plans with physicians.
• Maintain HIPAA-compliant records and complete OASIS documentation for RN.
• Uphold infection control standards and agency protocols.
