Qualifications

• Bachelor's degree in Social Work (BSW or MSW) from an accredited college or university.
• Current Kansas social work license in good standing (LBSW, LMSW, LSCSW, or other applicable social work license recognized by the Kansas Behavioral Sciences Regulatory Board).

Responsibilities

• Complete psychosocial assessments.
• Identify barriers affecting treatment, recovery, and discharge.
• Connect patients and families with community resources and financial assistance.
• Coordinate referrals for home health, hospice, rehabilitation, skilled nursing facilities, behavioral health services, durable medical equipment, and other post-acute services.
• Assist patients with advance directives.
• Participate in interdisciplinary care planning and discharge planning.
• Collaborate with physicians, nurses, therapists, Patient Financial Services, and community agencies.
• Promote safe, timely, patient-centered transitions of care.
• Maintain timely and accurate documentation in the electronic medical record.

Education
