Summary

Supervises the daily staff activities of Social Worker staff in addition to their normal social worker responsibilities. The Social Worker Supervisor provides patient education and emotional support, is responsible for referrals to community agencies, coordinates patient care with other team members, and assists with concrete services.

Responsibilities

Performs staff responsibilities to fulfill required service levels. Provides ongoing assistance with social needs to all patients. Responsible for the presentation of patients at selection committee as required by CMS, where appropriate.Oversees and is a resource to specialty social workers.Provides direction, orientation, training, coaching, and mentoring to staff. Performs or assists with performance evaluations and disciplinary actions.Assesses quality of services delivered and facilitates staff development programs. Ensures staff compliance with departmental and organizational policies, procedures, and protocols.
