Qualifications

• Master's degree in Social Work from an accredited program.
• Licensure in Michigan as a Licensed Social Worker.
• Experience in clinical social work within rehabilitation highly preferred.

Responsabilités

• Provides social work services to patients and families.
• Provides a diagnostic assessment of patient and family functioning and coping skills.
• Provides counseling/education to facilitate adjustment and problem solving.
• Acts as an internal case manager and liaison with external case managers.
• Coordinates transition from inpatient to outpatient services and community resources.
• Develops and implements a discharge plan.
• Documents patient/family status and progress per time standards.
• Communicates with disability team regarding needs and funding.
• Attends and participates in staff meetings and conferences.
• Advocates for patients and maintains confidentiality.
