LiveWell is seeking a Resident Support Specialist (Social Services) for our Long-Term Care team. The role focuses on guiding residents through admissions, creating personalized care plans, and connecting them with community resources to support medical, emotional, or financial needs.

You will work to develop trust, coordinate with families, and assist the Social Services Director in delivering meaningful programs while advocating for resident dignity and well-being.

Qualifications

• Bachelor’s degree in Social Work or related field is required.
• Experience in long-term care is a plus.
• Excellent written and spoken communication skills.
• Computer literacy and comfort with documentation.
• Compassionate mindset and positive attitude.

Responsibilities

What You’ll Bring:
