Mont Marie Rehabilitation and Healthcare Center is seeking a dedicated Social Worker to enhance the quality of life for our residents. This full-time position offers a salary ranging from $75,000 to $85,000 annually.

Qualifications

• 1+ years of experience as a Social Worker in a healthcare-related setting.
• Experience in a nursing home or rehabilitation center strongly preferred.
• Strong knowledge of regulations, policies, and guidelines governing nursing care facilities.

Responsibilities

• Evaluate current, new, and prospective residents to assess psychosocial needs.
• Address resident concerns and emotional needs while maintaining ongoing communication.
• Build and maintain positive relationships with residents and families.
• Coordinate care and collaborate with interdisciplinary teams.
• Develop and update individualized psychosocial care plans for residents.
• Participate in care planning meetings and support continuity of care.
• Provide counseling, emotional support, and resource referrals.
• Ensure compliance with facility policies, regulations, and documentation standards.
