Job summary

Barnes Jewish Hospital is seeking a Social Worker to support discharge planning, psychosocial assessments, and resource management. The role includes weekend discharge planning and weekday psychosocial evaluations, serving Memorial Belleville and Shiloh locations within the BJC system.

The ideal candidate holds a Master’s in Social Work and licensure, with preference for medical social work experience. Illinois license required or ability to obtain within 90 days.

Qualifications

• Master's Degree in Social Work is required.
• Medical Social Work experience is highly preferred.
• Illinois state social work license required or ability to pass licensing exam within 90 days of employment.

Responsibilities

• Provides direct clinical services to patients and families.
• Plans and implements social work services on assigned clinical service or unit in collaboration with the healthcare team.
• Collaborates with community human service agencies to meet the needs of patients, families, and the hospital.

Primary Responsibilities

• Weekends - Discharge Planning (coordination of DC plans)
• Weekdays - Psychosocial assessments, social determinants of health, resource management

• PRN - 2 shifts every 6 weeks (some telephonic on-call)
• Day Shift: 8am-430pm
• Location: Memorial Hospital (Belleville & Shiloh)

Minimum Requirements

• Master's Degree in Social Work
• Medical Social Work experience highly preferred

Overview

Memorial Hospital Belleville is an acute care hospital offering medical and surgical services plus critical and emergency care including Children's at Memorial for pediatric emergency care. It provides patients a full complement of diagnostic and treatment services as well as heart and vascular care. Memorial, offering medical and surgical services plus critical care, is an accredited Chest Pain Center with PCI by the Society of Cardiovascular Patient Care and is designated as an Acute Stroke Ready Hospital by the Illinois Department of Public Health. In addition, Memorial Belleville recently was accredited by the American College of Radiology as a designated Lung Cancer Screening Center. Since 2008, it has been designated as a Magnet®-recognized organization for nursing excellence by the American Nurses Credentialing Center.

Memorial Hospital Shiloh, a 94-bed, all-private suite hospital was recognized with Magnet status in 2018 and provides emergency care, labor & delivery, nursery, medical and surgical services plus critical care.

The Social Services Department enhances the delivery of comprehensive services to our patients. Staff provide information on discharge planning options such as home care, assisted living facilities, or nursing home placement. They can also discuss financial concerns and resources for assistance.

Preferred Qualifications

Role Purpose

Under direction of a physician, assists patients and their families in their residences or outpatient/inpatient settings to adapt and plan an interdisciplinary approach to treating the patient-family system. Employees working at an Illinois facility are required to have an Illinois state social work license or be able to take and pass the Illinois state social work licensing exam within 90 days of the first day of employment.
