• Experience in skilled nursing facilities is required.
• Strong ability to plan for pre-admission and discharge.
• Ability to assess psychosocial needs and contribute to care plans.

Responsibilities

• Plan for pre-admission and discharge.
• Provide psychosocial assessment and complete MDS sections A, AA, AB, AC, E, F, Q and B where applicable.
• Use MDS to guide the resident care plan.
• Respect and value the worth of each individual.
• Create an atmosphere of growth for the resident.
• Adopt a holistic view of well-being—social, psychological, physical, and spiritual.
• Ensure a supportive physical environment for residents.
• Foster a positive self-image through social contact and independence.
• Participate in resident care planning as part of the interdisciplinary team.

The social worker's role should be:

2. 2.Providing psycho-social assessment and completion of relevant parts of the minimum data set (particularly face sheets,
3. sections A, AA, AB, AC, E, F, Q and sometimes B) Utilizing the minimum data set to guide the care plan
4. 3.Is aware of the worth and uniqueness of each individual
5. 4.Treats each individual with respect
6. 5. Creates an atmosphere of growth for the individual
7. 6. Adopts a holistic perspective by recognizing the dynamic interplay of social, psychological, physical, and spiritual well-being
