Job summary

UnitedHealth Group is seeking a Part-Time Social Worker to join Elite Hospice, a nationwide care partner. This role provides medical social services under the supervision of a physician and an Interdisciplinary Group to address social and emotional factors influencing health.

This part-time position averages about 20 hours per week. The salary ranges from $56,440 to $84,660 annually for full-time employment, and a $5,000 sign-on bonus is offered to external candidates.

Qualifications

• Bachelor of Social Work or Master of Social Work required.
• Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
• Preferred: 1+ year of social work experience in healthcare.
• CPR certification or ability to obtain within 90 days.

As the Social Worker, provides medical social services under the direction of a physician and Interdisciplinary Group to assist in the understanding of significant social and emotional factors related to the patient's health status and in development of coping mechanisms.

This is a part-time position, expected to average about 20 hours a week.

Primary Responsibilities:

• Assists the patient, significant others, physician and health care team staff to understand significant personal, emotional, environmental and social factors related to the patient's health status on an as needed basis
• Contributes as a health care team member to the development of a comprehensive, integrated Plan of Care for patients on a daily basis
• Instructs health care team members on community resources available to assist patients on a as needed basis

Required Qualifications:

• Bachelor of Social Work OR Masters of Social Work

Preferred Qualifications:

• 1+ years of social work experience in a health care setting
• Current CPR certification or ability to complete within 90 days of hire
• Licensed Social Worker in the state of residence
