AccentCare Inc. in Columbia, MD is seeking a full-time Palliative Care Social Worker (MSW).
This on-site role provides patient- and family-centered social work services across hospital, home, and post-acute settings, with a Monday–Friday schedule. You will assess needs, coordinate care, support advance directives, discharge planning, grief counseling, and collaborate with the Palliative Care and Hospice teams.
