Responsibilities

• Complete admissions, social services evaluations, provide referrals, and communicate with residents and families.
• Assist guests with financial and legal matters, such as applying for pensions, and referring to lawyers or funeral homes for preplanning.
• Provide discharge planning services, including placement on waiting lists, arranging home care, and coordinating transfers.
• Arrange counseling services and psychiatric medication management as needed.
• Ensure accurate and timely documentation in MDS care plans.

Qualifications

• Current social work licensure in the state
• Bachelor’s degree in social work required; master's preferred
• One year of supervised social work experience in healthcare is preferred
