Job summary

Emanate Health in California is seeking a licensed MSW to conduct psychosocial assessments and develop patient discharge plans. The role supports the care team to address social, emotional, and psychological needs, and involves travel to multiple hospital worksites. Shifts are 8:00 AM–4:30 PM or 8:30 AM–5:00 PM.

Compensation is $43.00 per hour. Ideal candidates have MSW from an accredited program, hospital experience, and BLS certification, plus excellent customer service skills.

Qualifications

• MSW from an accredited school of Social Work.
• Minimum of 1 year experience working in a hospital environment is preferred.
• BLS certification required.
• Excellent customer service skills required.

Responsibilities

• Conduct psychosocial assessment to evaluate the patient’s current and future concerns.
• Develop discharge plans based on medical status and living circumstances.
• Minimize patient anxiety concerning illness and facilitate post-hospital referrals.
• Interact with and supplement the health care team to enhance social, emotional and psychological care of patient/family.
• Willing and able to travel to multiple worksites.
