Qualifications

• MSSW from an accredited school is required.
• 1 year post-master’s experience in health care (WA/TX) or 2 years (AK) preferred.
• Licensure as a Social Worker is preferred; BLS certification required for Home Health.

Responsibilities

• Assess psychosocial, financial, environmental, and community needs per social work plan.
• Provide direct and indirect social services support in homes, facilities, and hospitals.
• Deliver crisis intervention and supportive counseling.
• Complete documentation to comply with regulations and policies.
• Collaborate with interdisciplinary team to support care objectives.
• Attend team meetings and contribute to care planning.
• Provide consultation to colleagues, administration, and community partners.
• Support level-of-care changes and discharge planning.
• Respond promptly to referrals for social services needs.

Position Specific Responsibilities

• Assess patients’ psychosocial, financial, environmental, and community needs, as documented in the initial social work assessment, Plan of Care (POC), clinical records, and team meeting notes.


• Provide direct and indirect social services support for patients and families in all service settings (homes, long-term care facilities, hospitals).


• Deliver crisis intervention and supportive counseling for patients and families.


• Complete all documentation in compliance with federal/state regulations and policies to ensure regulatory compliance.


• Actively participate as a member of the interdisciplinary team, providing input to support patient/family care objectives.


• Attend team meetings, contribute to care planning, and collaborate with colleagues to coordinate patient/family services.


• Provide consultation and assistance to colleagues, administration, patients/families, and community partners as needed.


• Support level-of-care changes and discharge planning for patients and families.


• Respond promptly to referrals from the interdisciplinary team and community for social services needs.

• Promote teamwork, effectiveness, and efficiency through a cooperative and professional attitude.


• Education and/or Experience


• Required - Master's Degree from an accredited School of Social Work.


• WA/TX: Preferred - 1 year of post-Master’s experience in a health care setting.


• AK: Preferred - 2 years of supervised, post-Master’s experience in a psychiatric or other health care setting.

Certifications, Licenses, and Registrations

• Required (Home Health Only): BLS (American Heart Association, Vendor Managed).


• Preferred upon Hire: Social Worker License.

State Specific Requirements

• Advanced Social Worker License or Associate License; OR Clinical Independent Social Worker License or Associate License (Vendor Managed).
