Job summary

Providence Health Plan Group is seeking a Social Worker for the Spokane WA PACE program. You will provide social work services to participants with a focus on geriatric needs, in both individual and group settings, aligned with Providence's mission.

The role requires WA licensure for Social Worker, a MSW, and experience with diverse older adult populations. You’ll collaborate with an interdisciplinary team to promote independence and community living.

Qualifications

• Master's Degree in Social Work.
• WA Clinical Independent Social Worker License or Temporary License or Associate License upon hire.
• 2 years Social Work with geriatric age groups from diverse ethnic cultures.
• 1 year work experience with geriatric population.

Responsibilities

• Provide social work services to participants in direct service activities within the Center.
• Conduct one-on-one and group counseling and collaborate with interdisciplinary team.
• Support elders to address social issues and maintain independence in the community.

As a member of the interdisciplinary team, provides social work services to participants in a manner which is consistent with the mission and core values of Providence Health System. Spends a majority of time in direct service activities within the Center, including one-on-one and group counseling, interaction with other team members, and ongoing assistance to participants to address social issues.

Providence Home and Community Care (HCC) is a service line within Providence with over 8,000 caregivers and 2,000 volunteers serving nearly 200,000 people a year, in their homes, in clinics and in a variety of congregate living ministries across our seven-state footprint.
