Job description

Job Overview

This position oversees a range of clinical social work services to patients/clients and families in inpatient, ambulatory, and community settings utilizing individual, family, or group modalities and practicing within the guidelines of professional social work ethics and standards. The role includes participation in multi‑discipline case conferences, consultation with agency care providers regarding social service needs of clients and families, and collaboration with health care teams to promote coordinated plans of care.

Location

Community Based/Hybrid – Lawrence, Lowell, Malden, New Hampshire. Additional locations available.

Hours

Full‑time, 8:00 a.m. – 4:30 p.m.

Minimum Qualifications
  • Master’s degree in Social Work from an accredited institution.
  • Licensed Clinical Social Worker (LCSW); licensure in NH may be required depending on assigned territory and operational need.
  • One year of related experience.
Duties and Responsibilities
  • Provide psychosocial assessment and treatment planning for individuals and families, including psychiatric assessment according to DSM‑5 when appropriate.
  • Address crisis situations involving suicide risk, homicide, family violence, elder or child abuse.
  • Conduct long‑term care assessments, develop in‑home care plans, and arrange placement when necessary.
  • Offer therapy, psycho‑education, and counseling in individual, family, or group modalities.
  • Assist patients/families with referrals, resource acquisition, and understanding of psychosocial factors that influence care utilization.
  • Advise the health care team on psychosocial contributions to illness, treatment, and recovery, and recommend strategies to address identified issues.
  • Provide medical social services to family members or caregivers on a short‑term basis when needed.
  • Treat patients and families with dignity and respect, fostering open communication and empathy.
  • Conduct outreach, case‑finding, and screening for high‑risk issues affecting patient progress.
  • Assist with discharge planning, ensuring continuity of care and implementation of outpatient psychosocial components.
  • Collaborate with social, governmental, and legal agencies, courts, schools, clinics, hospitals, physicians, and other partners.
  • Act as a patient/family advocate and, when required, testify in court.
  • Maintain comprehensive clinical documentation and complete administrative record‑keeping, including billing, evaluations, and performance reports.
  • Initiate and support policy and program development in specific service areas or teams.
  • Engage in departmental meetings, committees, and community representation.
  • Participate in professional education, supervision, and teaching activities.
  • Collaborate with peers to achieve team goals and foster a positive work environment.
  • Monitor, assess, and modify practice based on quality improvement initiatives and research.
  • Contribute to professional organizations through grant writing, teaching, presentations, seminars, and workshops.
  • Ensure compliance with regulatory guidelines (DPH, ERISA, HIPAA/HITECH) and confidentiality principles.
