Capital Health (US) is seeking a full-time Social Worker for case management. This role requires a Master’s degree in Social Work (MSW) and at least one year of experience in healthcare. The Social Worker will assess patients’ clinical and social needs, working with a multidisciplinary team to facilitate timely discharges.

Pay Range: $34.99 - $45.71 per hour.
Scheduled Weekly Hours: 40
Position Type: full‑time equivalent (1.0 FTE). Compensation may vary with hourly assignment or part‑time status.
Assesses, intervenes, and plans appropriate discharge based on patient’s clinical, social, and functional needs. Integrates assessment using cultural, clinical, ethnic, age and other factors into the plan of care to implement transition plans. Acts as an active member of the care management team in the discharge planning process identifying complicated social and financial factors and barriers to appropriate and timely discharge. Refers patients, families, and caregivers to resources for abuse/neglect/domestic violence, alcohol and substance abuse.
Education: Master’s degree in Social Work (MSW).
Experience: One year in hospital case management or health care.
Other Credentials: Licensed Social Worker.
Continuing Education: 4 hours of stroke‑related contact hours annually if assigned to indicated units (Critical Care, Intermediate Care, Emergency, Neuro, Cardiology, Peds ED, PACU, Interventional Radiology, CNI, Observation).
Excellent communication, conflict management, organizational and planning skills.
Clinical experience in a medical environment; knowledge of spreadsheet and word processing software, case management and patient information systems.
Comfortable working with diverse age ranges and populations.
Frequent physical demands include: Standing, walking, carrying objects, keyboard use, repetitive motion.
Occasional physical demands include: Sitting, climbing (stairs or ladders), pushing/pulling, twisting, bending, reaching forward, reaching overhead, squatting/kneeling/crawling, wrist positions, pinching/fine motor activities.
Continuous physical demands include: Lifting floor to waist 15 lbs; lifting waist level and above 10 lbs.
Sensory requirements include: Bloodborne pathogens, airborne communicable disease.
