Job summary

The VIOS Clinic is seeking a Hospice Medical Social Worker to join the interdisciplinary team and provide emotional, social, financial, and resource support to patients and families facing serious illness and end-of-life care.

You will conduct social casework, assess financial needs, collaborate with external agencies, and help coordinate Age-Friendly Care by addressing What Matters, Medications, Mentation, and Mobility.

Qualifications

• MSW from an accredited School of Social Work.
• Current state Social Worker License.
• Proficiency in clinical skills, communication, and documentation.
• Valid driver’s license with good driving record.
• Availability during agency operating hours.

Responsibilities

• Conduct social casework and assess families' financial situations, referring to resources as needed.

Preferred Experience & Certifications

• Advanced Palliative Hospice Social Worker Certification (APHSW-C).
• At least two years of hospice experience.
• Certifications such as CVA and EMR experience are advantageous.
