Fairmount Behavioral Health System seeks a PRN Social Worker/Case Manager to provide psychosocial assessments, therapy, and discharge planning on a designated unit within a leading behavioral health facility.

The role involves crisis intervention, liaison work with referral sources, and collaboration with treatment teams to support patients and families in psychiatric and substance use contexts.

Qualifications

• Master’s Degree in Behavioral Healthcare required; Master’s degree in Social Work preferred
• PA License preferred
• Knowledge of case management, discharge planning, growth and development, crisis and behavior management, therapy theory/practice
• 1–3 years psychiatric behavioral health experience required, 3–5 years preferred

Responsibilities

• Provide psychosocial assessments, individual, group and family therapy
• Participate in treatment team planning and discharge planning/case management
• Coordinate crisis intervention and liaison services with referral sources and hospital departments
• Process commitments and support services across designated unit

We are currently seeking a Weekday PRNSocial Worker/Case Managerto join our team.

The PRN Social Worker/Case Manager provides clinical social work/case management services on a designated unit in a manner consistent with the philosophy and objectives of Fairmount Behavioral Health System, the NASW and if licensed, the licensing board of Pennsylvania. The PRN Social Worker/Case Manager is responsible for psychosocial assessments, individual, group and family therapy, participation in treatment team planning, processing commitments, discharge planning/case management service, crisis intervention and liaison services with referral sources and other departments within the hospital.

Qualifications

• Master’s Degree in a Behavioral Healthcare field required, Master’s degree in Social Work preferred
• PA License preferred
• Must possess knowledge of case management, discharge planning, age-specific growth and development, crisis and behavior management, individual and family therapy theory and practice, and behavioral health/substance use
• Prior experience working with a psychiatric behavioral health population between 1-3 years required, 3-5 years preferred
