Elderwood of Lakeside at Brockport is seeking a Social Worker to support residents in our long-term care community. The role involves identifying psychosocial, mental and emotional needs, linking residents to services, and guiding admissions, care planning, transitions and discharge decisions.

Qualifications

  • Bachelor’s degree in social work from an accredited institution; or a bachelor’s degree in a related human-services field, including human rehabilitation counseling, psychology, sociology, gerontology or special education
  • At least one year of supervised social work experience in a healthcare setting working directly with individuals
  • Casework experience
  • Experience supporting admissions or discharges is strongly preferred
  • Valid driver’s license and a clean driving record
  • Ability to work effectively with older adults, families and interdisciplinary team members
  • Strong interpersonal, communication, organization and documentation skills
  • Ability to read, write, speak and understand English at an intermediate or higher level to support safe, effective workplace communication

Responsibilities

  • Collaborate with nursing, rehabilitation and other departments to address residents’ physical, emotional and social needs
  • Provide individual, family or group support within the scope of the position
  • Participate in staff education and in-service programs as requested
