Qualifications

• Licensed Master Social Worker (LSW) in Louisiana.
• Clinical social work experience in hospital or clinical settings preferred.
• Ability to assess psychosocial needs and coordinate discharge planning.

Essential Functions

The Social Worker provides clinical social work services for patients and their families. Responsible for psychosocial assessment, discharge planning for patients with complex psychosocial and medical problems. Assists patients and their families in coping with difficulties related to hospital admission, illness, treatment and discharge.
