Job summary

Enloe Health Rehabilitation Center, part of Lifepoint Health, is seeking a licensed Social Worker to join our 40-bed acute rehabilitation unit. You will interview patients and families, coordinate services, assist with discharge planning, and document progress within established timelines.

Required: Master's degree in Social Work and California LMSW license, BLS-HCP, and Crisis Intervention Training within 90 days.

Qualifications

• Master's degree in Social Work required.
• CA LMSW licensure required.
• BLS-Healthcare Provider certification required, Crisis Intervention Training within 90 days of hire/transfer.
• Ability to think critically and work with minimal supervision.
• Ability to perform well in a high-stress environment.

Responsibilities

• Interviews and assesses patients and families, caregivers, or legal representatives.
• Determines and arranges needed internal and external services/interventions.
• Participates in case reviews and coordinates with the healthcare team to monitor treatment plans.
• Assists with discharge planning and identifies resources for transportation, housing, healthcare, and social services.
• Performs Medicaid screenings as indicated for program participants.
• Completes required documentation within specified timeframes.

Enloe Health Rehabilitation Center is a 40-bed acute rehabilitation unit dedicated to helping patients who are recovering from various injuries or illnesses. In partnership with Lifepoint Rehabilitation, this facility provides 24 hours of nursing care and intensive inpatient therapy to provide an outstanding experience and quality outcomes for all of the patients we serve.

How You'll Contribute

A Social Worker who excels in this role:

• Interviews and assesses patients and/or patient's family, caregivers, and/or legal representatives.
• Determines, prioritizes, provides and/or arranges for needed internal and external services/interventions.
• Participates in case reviews to evaluate case management and progress. Consults with healthcare team members to promote, monitor, and evaluate compliance with patient's treatment plan.
• Assists with discharge planning and processes. Identifies appropriate resources, including transportation, housing, healthcare, and social/spiritual services, and provides referrals as part of the discharge plan.
• Performs Medicaid screenings as indicated for NHP.
• Completes required documentation within required time frames.

What We're Looking For

• Education: Master's degree Required
• Licenses: State of California Licensed Master's Social Worker (LMSW)
• Certifications: Basic Life Support Health Care Provider (BLS-HCP); Crisis Intervention Training (within 90 days hire/transfer) 2 years related experience
• Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision.
• Must be able to work in a stressful environment and take appropriate action

Pay wage: $32.64-$59.16
