Job Summary

Schedule: Sat and Sun 8‑hr shifts, 8:00 a.m. – 4:30 p.m., plus one additional 8‑hr shift during the week, either Monday or Friday. Licensed Master Social Worker systematically intervenes to provide clinical social work and complex discharge planning to patients and their families with complex psychosocial needs. Under supervision of a licensed clinical social worker, offer crisis intervention and/or mental health assessment to patients and families, coordinate and facilitate the development of multidisciplinary discharge plans for high‑risk patient populations. Reports to Manager or Director, Case Management.

Minimum Qualifications

• Graduate of an accredited Masters of Social Work (MSW) program.
• Current license as a Master Social Worker (LMSW) in Texas.
• ACM certification from American Case Management Association (preferred).
• Field placement or internship experience in health services/health care provider, with acute inpatient hospital social work experience preferred.
• Effective oral and written communication skills.
• Knowledge of DSM‑5 and ICD‑10.
• Knowledge and skill in social work assessment and treatment of patients for mental health status and substance abuse screening.
• Excellent therapeutic communication and negotiation skills.
• Strong analytical skills.
• Experience in utilization management, managed care, and payer issues.
• Experience in pre‑acute and post‑acute care and community resources.
• Ability to work independently and develop collaborative relations with physicians, families, patients, interdisciplinary team, and community agencies.

Principal Accountabilities

• Assess patient and family psychosocial risk factors through evaluation of prior functioning levels, support systems, reaction to illness, and coping ability.
• Develop and carry out treatment plans using clinical social work diagnoses, assessments, and interventions as part of a multidisciplinary team.
• Intervene with patients and families regarding emotional, social, and financial consequences of illness or disability, and mobilize family/community resources to meet identified needs.
• Provide crisis intervention, short‑term individual, marital, and family therapies under supervision.
• Offer intervention in cases involving child abuse/neglect, domestic violence, guardianship, institutional abuse, foster care, adoption, mental health placement, advance directives, adult/elderly abuse, child protection, and sexual assault.
• Serve as a resource person and provide counseling and intervention related to treatment decisions and end‑of‑life issues.
• Advocate for patient and family empowerment and independence in healthcare decisions and accessing services.
• Participate in discharge planning for complex patients to ensure timely discharge and appropriate linkage with post‑discharge care providers.
• Communicate with clinical care team members about discharge status and coordinate with significant community resources.
• Consult with case managers when coordination with substantial community resources is needed to achieve desired outcomes.
• Provide transition planning and referrals to post‑acute care providers.
• Keep clinical and case management team updated on post‑acute provider acceptance and clearance for discharge.
• Validate discharge criteria for patient and families, identifying new resources or changes in previously identified resources.
• Educate patients/families and physicians regarding post‑acute options and address choices.
• Ensure safe care to patients, staff, and visitors; adhere to all Memorial Hermann policies, procedures, and standards within budgetary limits.
• Promote professional growth by meeting mandatory continuing education and competency requirements; support department‑based goals.
• Act as preceptor, mentor, and resource to less experienced staff.
• Perform other duties as assigned.
