Job summary

Providence Santa Rosa is seeking a per diem/on-call Social Worker/Clinical Counselor to provide psychosocial assessment and counseling to terminally-ill patients and families, with coverage for Healdsburg and Petaluma Valley as needed. You will work as part of a care team to explore options, support decision making, and connect patients with services.

A California Clinical Social Worker license and healthcare experience are required.

Qualifications

• Master's Degree in Social Work, Psychology or Counseling; PhD preferred.
• California Clinical Social Worker License upon hire.
• 1 year of healthcare or related field experience.
• Training in family counseling and case management.

This is a per diem/on call inpatient position supporting patients in Santa Rosa Hospital but will be supporting Healdsburg and Petaluma Valley as needed.

Required Qualifications:

• Master's Degree - Social Work, Psychology or Counseling. Graduate from an accredited school preferred or,
• Ph.D. -Social Work, Psychology or Counseling. Graduate from an accredited school preferred.
• Coursework/Training - Training in family counseling and case management.
• California Clinical Social Worker License upon hire.
• 1 year - Experience in a health care setting or related field.

The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
