Providence is seeking a per diem/on-call inpatient Social Worker to support patients in Santa Rosa Hospital and nearby facilities as needed. You will provide psychosocial assessments, counseling and emotional support to terminally ill patients and families, collaborating with the team to explore options and problem solve.

Qualified candidates hold a California Clinical Social Worker License and have 1 year of healthcare experience. A comprehensive benefits package accompanies the role.

Qualifications

• Master's degree in social work, psychology or counseling.
• Ph.D. in social work, psychology or counseling preferred.
• California Clinical Social Worker License required on hire.
• 1 year of healthcare or related field experience.

Responsabilités

• Perform psychosocial assessment and provide counseling, social work and emotional support to terminally-ill patients or those in palliative care.
• Collaborate with the care team to identify problems and aid patients/families in exploring options and problem solving.
• Work as part of Providence’s team to deliver patient-focused, whole-person care.
• Support patients across Santa Rosa Hospital, Healdsburg, and Petaluma Valley as needed.

Description

This is a per diem/on call inpatient position supporting patients in Santa Rosa Hospital but will be supporting Healdsburg and Petaluma Valley as needed.

Provides psychosocial assessment and delivery of counseling, social work and emotional support services to terminally-ill patients, and/or palliative services to patients and their families. Works as a team member to identify problems and assist patient/family members in exploring realistic options, decision making and problem solving.

Required Qualifications

• Master's Degree - Social Work, Psychology or Counseling. Graduate from an accredited school preferred or,
• Ph.D. -Social Work, Psychology or Counseling. Graduate from an accredited school preferred.
• Coursework/Training - Training in family counseling and case management.
• California Clinical Social Worker License upon hire.
• 1 year - Experience in a health care setting or related field.

Posted are the minimum and the maximum wage rates on the wage range for this position. The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
