Job summary

Providence Saint John's Medical Foundation is seeking a behavioral health care manager who collaborates with the patient's medical provider and psychiatric consultant to coordinate mental and physical health care for an assigned caseload.

You will support a collaborative care team within a primary care setting, applying evidence-based counseling techniques and guiding care planning to improve outcomes for patients.

Qualifications

• Bachelor's degree required.
• Master's degree in Social Work required.
• California Clinical Social Worker License at hire.
• Minimum 2 years in inpatient, outpatient, or home-based social work.
• Experience with counseling and group work.
• Experience with screening for mental health and/or substance use disorders.
• Experience with assessment and treatment planning for mental health/substance use disorders.
• Experience with evidence-based counseling techniques.

The behavioral health care manager is a core member of the collaborative care team, including the patient's medical provider and psychiatric consultant, as well as the larger primary care team or medical team. The behavioral health care manager is responsible for supporting and coordinating the mental and physical health care of patients on an assigned patient caseload with the patient's medical provider and, when appropriate, other mental health providers.

Required Qualifications:

• Bachelor's Degree.
