Community Hospice & Palliative Care is seeking a Social Worker to provide psychosocial services to patients and families supported by the IDG. The role includes counseling, resource procurement, and coordinating care across home care, long-term care, and assisted living settings.
Responsibilities include managing complex cases, assessing needs, educating patients and families, and collaborating with NASW standards in palliative care while participating in interdisciplinary team meetings.

The Social Worker provides social services and support to patients and their family members. A contributing member to each patient’s Interdisciplinary Group (IDG), the social worker provides psychosocial interventions, referrals, assessments, supportive counseling, networking, and resource procurement to patients and families.
Visits patients in various settings, to include but not be limited to: hospice inpatient units, residential homes, long term care or assisted living facilities, group homes, shelters, or other locations where services to patients/families may need to occur.
