Job summary

Tri-Counties Regional Center seeks a Service Coordinator 2 to provide person-centered consultation, advocacy and service coordination for people with developmental disabilities who require specialized support.

The role emphasizes collaboration with families, interdisciplinary teams and local agencies to secure services, plan outcomes and improve community integration. Strong knowledge of local resources and Spanish/English communication are required.

Qualifications

  • Master’s or Bachelor’s degree in human services or related field plus relevant experience.
  • 2 years case management experience desirable in mental health or social services.

Responsibilities

  • Conduct strength-based assessments and involve families in person-centered planning.
  • Coordinate and monitor services and supports with IDT teams.
  • Provide information and referrals to community resources and agencies.
  • Develop relationships with vendors and community organizations.
  • Maintain regular contact with individuals served and update IFSP/IPP.

Job Summary:

Within a team environment, provides person-centered consultation, advocacy, resource information and service coordination for people with developmental disabilities who require more involved and specialized assistance.

People with developmental disabilities served by Service Coordinator 2’s require specialized assistance, including but not limited to:
  • Students in active transition due to graduating from school in one to two years.
  • Individuals in residential placement or living independently.
  • Newly eligible individuals, transfers from other Regional Centers, and reactivated cases.
  • Individuals with dual diagnosis, and cases requiring clinical expertise.
  • Self-determination and Medicaid Waiver cases, Social Security/Medi-Cal problems that require assistance with advocacy and appeal process.
Specific Job Duties:
  • Support individuals we serve by maintaining regular contact and doing Individual Family Service Plans (IFSP) and Individual Program Plans (IPP).
  • Participate in Interdisciplinary Team (IDT) staffings and attend meetings in the community as needed including Individual Education Plan (IEP) and day program meetings.
  • Provide information and referral to generic services and resources in the community, including
    • Schools, recreational options and local support groups.
    • Supportive Employment/Habilitation funded day programs.
    • Local agencies (C.C.S., Dept. Rehabilitation, Social Security, Medi-Cal, IHSS).
    • Specialist physicians/ specialists (speech therapist, O.T./P.T.).
  • Coordinate and secure TCRC funded services, including
    • Behavior services, training for independent living, supportive living.
    • Out-of-home placement, in home and out-of home respite.
    • Infant and adult day programs.
    • Durable medical equipment and specialized services, non-Medi-Cal funded services.

Minimum Qualifications:
  • Master’s or Bachelor’s degree in human services field or a related area, with 2 years of applicable experience in mental health or social work.
  • 2 years experience in case management within a related field.
  • 1 year experience with individuals with developmental disabilities preferred.
  • Valid California driver’s license and reliable, insured vehicle or equivalent means of transportation.
  • Able to travel to any location at any time.
  • Able to read/speak/write Spanish and English Required
Physical Requirements:

Position requires the ability to work independently. Must possess mobility to work in a standard office setting and use standard office equipment, including a computer. This is primarily a sedentary office classification. The position also requires grasping, repetitive hand movement, finger dexterity and fine coordination in preparing statistical reports and data using a computer keyboard and calculator.

Additionally, the position requires near, far, and color vision in reading correspondence, statistical data, and using the computer. Hearing and speech are required to communicate in person, before groups and over the telephone.

Employees must possess the ability to lift, carry, push and pull materials and objects necessary to perform job functions. The employee must frequently lift and/or move up to 10 lbs. and infrequently up to 20 lbs. The position requires standing, walking, reaching, twisting, turning, kneeling, bending, squatting, and stooping in the performance of daily office activities. Work is performed in an office setting with moderate noise levels, controlled temperature conditions and no direct exposure to hazardous physical substances. Employees may encounter upset individuals over the phone and/or in person. Each of these physical tasks is an essential function of the position.

Bachelor's degree: Starting Salary: $27.55 - $30.36 hourly. Full Salary Range: $27.55 - $33.53

Master’s degree: Starting Salary: $31.90 - $35.16 hourly. Full Salary Range: $31.90 - $39.89
