ChristianaCare in Newark, Delaware is seeking a dedicated Social Worker to support care coordination and discharge planning within our Care Management Model. The role emphasizes patient-centered care, collaboration with the RN Case Manager and Utilization Management to ensure appropriate disposition and payer coverage.

This position requires a MSW or MSS and state licensure (CSW or MSW) with acute care experience preferred.

Formación

• The successful candidate may be eligible for a $15,000 sign on bonus to be paid within first year of a two-year commitment.

THE CARE MANAGEMENT MODEL:

E D U C T I O N AND EXPERIENCE REQUIREMENTS:

• Master's degree in Social Work (MSW) or Master of Social Services degree (MSS) from an accredited graduate school required prior to hire date.
• Licensed CSW or MSW as required by the state of Delaware prior to hire date.
• Acute Care Social Work experience preferred.

S P E C I A L R E Q U I R E M E N T S:

CCM preferred within 18 months of eligibility.

Hourly Pay Range: $30.95 - $49.52This pay rate/range represents ChristianaCare's good faith and reasonable estimate of compensation at the time of posting. The actual salary within this range offered to a successful candidate will depend on individual factors including without limitation skills, relevant experience, and qualifications as they relate to specific job requirements.
