Job summary

Department of Human Services in Illinois is seeking a Social Worker II for the Shapiro Center in Kankakee to provide professional social work services and discharge planning for individuals with intellectual disabilities.

You will work as part of an interdisciplinary team, handle case management, funding applications, and community agency coordination to ensure quality care and safety.

Qualifications

• Master’s degree in social work from a recognized college or university.
• One (1) year of professional social work experience in the evaluation and treatment/habilitation of mentally ill, developmentally disabled, or other disabled persons.
• This class is included as an Upward Mobility Program credential title.

Responsibilities

• Performs professional social work services in treatment of individuals with intellectual disabilities on an assigned unit.
• Provides case management, including completing funding applications and liaison between Shapiro Center and community agencies.
• Develops discharge plans with the interdisciplinary team according to clinical and social needs assessments and in collaboration with community agencies.
• Participates in facility team meetings and prepares reports, forms, and correspondence in a timely manner.
• Assists family/guardian with understanding financial information and services provided at Shapiro Center and by community agencies.
• Performs other duties within the scope of the listed responsibilities.
