Job summary

Atrium Health Wake Forest Baptist is seeking a Full-time Social Worker in Case Management. This role focuses on identifying patients needing psychosocial support, assessing needs, and coordinating discharge planning within a multidisciplinary team.

The ideal candidate holds a CSWE-accredited MSW and may pursue licensure; ACM-SW certification is preferred. The position is based in Winston-Salem, NC with a daytime Monday–Friday schedule.

Qualifications

  • Master's degree in Social Work from a CSWE-accredited program.
  • Clinical licensure desirable; ACM-SW preferred.

Responsibilities

  • Identify patients who would benefit from Case Management interventions.
  • Assess data by interviewing patients and evaluating needs.
  • Complete psychosocial assessments per standards to identify emotional and social needs.
  • Develop and implement psychosocial treatment plans with appropriate modalities.
  • Initiate referrals to internal and external services as needed.
  • Maintain knowledge of payer and reimbursement practices affecting care plans.
  • Coordinate care conferences to drive patient progression.

Education / Experience

Master's degree in Social Work from a school accredited by the Council on Social Work Education (CSWE)

Licensure, Certification, And/or Registration

Clinical licensure (or provisional status) desirable Certification as an Accredited Case Manager preferred (ACM-SW) preferred

Essential Functions
  • Identifies patients who would benefit from Case Management interventions based on an initial screening assessment of discharge needs.
  • Assesses all relevant data and obtains information by interviewing patient/family and performing objective evaluation of patient needs.
  • Completes psychosocial assessments in accordance with departmental and professional standards to identify emotional, social and environmental needs related to diagnosis, illnesses, treatment and life situations.
  • Formulates, develops and implements a psychosocial treatment plan utilizing appropriate social work modalities and interventions, which may include crisis intervention, individual and family therapies, and grief and bereavement counseling.
  • Initiates appropriate internal and external referrals specific to individual patient needs.
  • Maintains current knowledge of available federal, state, and local regulations. 7. Advocates for unmet needs on behalf of patients.
  • Participates in the development of a discharge plan in collaboration with the multidisciplinary team to drive patient progression through the continuum of care.
  • Maintains current knowledge and awareness of payer and reimbursement practices impacting the plan of care.
  • Coordinates patient and family care conferences as needed.
  • Identifies barriers to efficient and effective management of patient care and seeks strategies to eliminate the barriers
  • Documents in in the Electronic Medical Record in accordance with departmental reporting standards.
  • Maintains working knowledge of payer and reimbursement practices impacting the plan of care. Demonstrates the ability to guide the patient and family through an evaluation of their options for post discharge care.

Provide social work services, complex discharge planning, and care coordination to patients and their families. Identify needed community services and refer patients in need to the appropriate services. Collaborate with other members of the multi-disciplinary team to affect positive patient and family outcomes. Communicate with other hospital staff re: psycho-social issues that impact health condition/status.
