Under the supervision of the Specialized Health and Social Services (SHSS) program supervisor, this role provides complex and comprehensive public health social work and case management services for children with special health care needs and their families, high-risk pregnant women, and the general public. Responsibilities include conducting home visits, completing intake and assessment documents, collaborating with medical providers and community partners, monitoring and following up on cases, maintaining accurate documentation, conducting presentations, attending community events, and performing duties as assigned. The role also supports incident command structure in public health emergencies and complies with HHS, DSHS, and Texas Department of Public Safety policies.
Current, unrestricted license to practice social work in the State of Texas.
Bachelor’s or graduate degree from a Council on Social Work Education accredited college or university.
Master's Degree preferred.
Bilingual (Spanish) preferred.
