• Assessing the social, emotional, and psychological needs of residents and identifying opportunities for improvement
• Developing and implementing individualized care plans to ensure residents receive comprehensive care
• Providing counseling, guidance, and support to residents and their families
• Collaborating with other healthcare professionals to ensure a coordinated approach to care
• Identifying and addressing social determinants of health that impact residents' overall well-being
• Conduct initial and ongoing assessments of residents' social, emotional, and psychological needs
• Develop and implement individualized care plans in collaboration with the interdisciplinary care team
• Provide counseling, guidance, and support to residents and their families
