Job summary

MJHS is a large not-for-profit health system serving the Greater New York area, offering hospice and palliative care, home care, and related services. Qualifications

• Graduate from an accredited School of social work.
• Minimum of one year experience in Social Work in a healthcare setting.
• Knowledge of Word and Excel preferred.
• New York State MSW licensure.
• Valid NYS Drivers license and car required.
• Ability to work within an interdisciplinary team environment.
• Ability to input clinical data proficiently.

Responsibilities

• Provide psychosocial support and other Social Work services to Hospice patients and families in inpatient, home care, or nursing home settings.
• Serve as social work member of hospice IDG and as liaison between hospice staff and inpatient and nursing home staff.
• Develop social work plan of care, goals and recommendations based on patient/family needs for assigned patients.
• Develop social work plan of care and discharge plans when appropriate for patients admitted.

Fee for Service rates:

LMSW rate: $43 per hour

LCSW rate: $46 per hour

In-Person Patient Evaluation: $60 per visit (Telehealth: $40/visit)

Pediatric Specialty: $69 per visit (Telehealth: $40/visit)

Orientation (classroom) $165.00 per day

• You will provide psychosocial support and other Social Work services to Hospice patients and their families in the inpatient, home care/nursing home settings, in pre and post bereavement phases of Hospice service
• You will serve as social work member of hospice IDG and as liaison between hospice staff and inpatient and nursing home staff
• You will develop social work plan of care, goals and makes recommendations based on patient/family needs for assigned patients who are direct admission to inpatient units or nursing home/hospice residents.
• You will develop social work plan of care and discharge plans when appropriate for patients admitted into inpatient facility.
