Job summary

Catholic Home Health Services is seeking a qualified Social Worker to provide direct and intermittent home-based services. You will support patients and families, contribute to Plans of Care, and coordinate with the healthcare team to enhance overall well-being.

The role requires an MSW, an active Florida license, and 1–2 years in healthcare social work, preferably in home health. Strong English skills and computer literacy are essential.

Qualifications

• Master's Degree in Social Work from an accredited program.
• Active Florida social work license (LSW/LSW-AP/ similar).
• 1–2 years of social work experience in healthcare; home health preferred.
• Knowledge of AHCA regulations and Medicare COPs is preferred.
• Ability to read, write and understand English; computer skills required.

Responsibilities

• Provide social work services primarily in patients' homes on a visiting basis.
• Participate in development and re-evaluation of physician Plan of Care (POC).
• Instruct and counsel patients and families on coping with illness and psychosocial issues.
• Perform psychosocial assessments of patients and families.
• Apply infection control practices and maintain patient confidentiality.

HOURS: MONDAY TO FRIDAY 8AM TO 5PM (ALTERNATING WEEKENDS)

Summary & Objective

The Social Worker provides direct and indirect social worker services primarily on a visiting and intermittent basis in the patient's home in accordance with the policies and procedures of Catholic Home Health Services.

Essential Functions

• Instruct, treat, observe and evaluate patients exhibiting significant social and emotional problems affecting their health status.
• Participate in the development and periodic re-evaluation of the physician's Plan of Care (POC).
• Instruct and counsel patients and families in treating and coping with social and emotional response connected with the illness.
• Performs assessments in patient and families to determine psychosocial needs.
• Assess the pre-bereavement/anticipatory grief needs.
• Implement the Plan of Care provisions to meet their needs, which include: social service goals for alleviating problems, supportive counseling, problem solving, community referrals, etc
• Apply concepts of agency's infection control plan and universal precaution in coordination/performing client cares activities to protect both patient and staff (OSHA).

Other Duties

• Complete appropriate documentation in a timely manner to assure compliance with agency policy.
• Demonstrate commitment and professional growth by participating in in-service programs and maintaining/improving competency.
• Maintain the highest standards of professional conduct in relation to information that is confidential in nature. Share information only when recipient's right to access is clearly established and sharing of such information is in the best interest of the patient.
• Meet mandatory continuing education requirements of the agency/licensing board
• Maintain your required licenses, certifications and mandatory skill updates.
• Promotes positive employee engagement, teamwork, mutual respect and safety work practices.
• Comply with all policies, local, state and federal laws and regulations.
• Provide other duties of healthcare team member.
• Perform other duties as assigned.

Supervisory Responsibility

• May serve as an interim leader depending on need

Physical Requirements

• Must be able to lift and/or move up to 50 pounds and push/pull up to 250+ pounds, walk, climb stair or ladders, stand on feet for extended periods of time, etc.

Knowledge & Experience Requirements

• Master's Degree from a school of Social Work accredited by the Council on Social Work Education.
• Clear and Active Professional License to practice in the state of Florida.
• 1-2 years of social work experience in a health care setting. Home care experience preferred.
• Knowledge of AHCA Regulations, Medicare COP's Guidelines required.
• Knowledge of public health administration and/or social work consultation and/or supervision preferred.
• Must have knowledge of computer office/clinical software.
• Must be able to read, write and understand the English language.
