Position Summary:

The Social Worker provides trauma-informed crisis response, care coordination, and hope-centered support to individuals and families impacted by community violence. This role coordinates trauma response in the aftermath of shootings or other violent incidents, supports participant stabilization, and ensures continuity of care across medical, behavioral health, and community-based services. In collaboration with Violence Intervention Specialists, the Social Worker delivers non-clinical, hope-centered individual support and co-facilitates structured support groups, including groups focused on historical and intergenerational trauma, to promote healing, insight, and non-violent pathways forward. The Social Worker also coordinates transportation and access to essential services, participates in community-led responses to violence, and contributes to a multidisciplinary team approach aimed at reducing violence and strengthening hope, agency, and long-term stability.

Duties and Responsibilities:

Outreach and Engagement

• Engage high-risk individuals in the community using street-level presence and building rapport.
• Represent the program through consistent presence in the community and participate in outreach and community mobilization objectives.
• Support and participate in community-led responses to violence, including healing circles, vigils, peace walks, and awareness events following violent incidents.
• Assist in planning and implementing community mobilization activities that promote non-violence, healing, and collective responsibility for safety.
• Engage residents, participants, families, and community partners to strengthen collective efficacy and community ownership of violence prevention and intervention efforts.

Trauma Response and Crisis Support

• Coordinate trauma response in the aftermath of shootings or violent incidents involving participants, families, or close associates
• Oversee resiliency response team of volunteers
• Assess immediate safety concerns, trauma symptoms, and risk of retaliation
• Collaborate with Violence Intervention Specialists during crisis response and follow-up engagement
• Provide Psychological First Aid to participants, family members, and close associates following violent incidents, including triage, emotional stabilization, and practical support.

Assessment and Care Coordination

• Conduct biopsychosocial intakes and ongoing assessments to identify participant needs, strengths, and risk factors
• Provide care coordination tailored to assessed risk level, including intensive support for individuals at elevated risk of continued violence involvement
• Coordinate and monitor referrals to medical care, behavioral health services, substance use treatment, housing, and other social supports
• Coordinate and, when appropriate, provide transportation to medical appointments, court obligations, and other essential services
• Ensure continuity of care following violent incidents, when applicable

Hope-Centered Individual and Group Engagement

• Provide non-clinical, hope-centered individual support focused on strengthening goals, pathways, and agency
• Support participants in identifying future-oriented goals and practical steps toward stability and safety
• Co-facilitate structured support groups, including historical and intergenerational trauma-focused groups, in collaboration with program staff
• Support psychoeducation, guided reflection, and healing-centered dialogue in a safe, respectful environment
• Work collaboratively with Violence Intervention Specialists, Team Leads, and program leadership to align trauma response and care coordination efforts
• Participate in community-led responses to violence, including healing circles, vigils, and prevention activities
• Build and maintain relationships with community providers to strengthen referral pathways and coordinated care

Documentation & Compliance

• Maintain timely, accurate, and complete documentation, records, and reports in compliance with program, hospital, and grant requirements.
• Contribute to data collection and reporting related to participant outcomes and service utilization
• Adhere to program goals, procedures, methods, and interventions as outlined in the grant agreements.
• Engage in ongoing professional development opportunities that strengthen trauma-informed and violence intervention skills, while fostering personal growth and career advancement.
• Participate in reflective supervision and team meetings designed to build resilience, manage vicarious trauma, and promote a culture of mutual support.
• Contribute to a learning environment where staff share insights, strategies, and innovations to improve participant outcomes and advance the field of violence intervention.
• Set and pursue at least one annual professional goal in collaboration with a supervisor, supporting long-term growth.
• Support a team culture grounded in hope, collaboration, and trauma-informed centered engagement, modeling the values we promote with participants.

Requirements:

• Licensed Social Worker or eligibility for licensure preferred but not required.
• Minimum of 2 years' experience working with individuals impacted by violence, trauma, or the criminal justice system.
• Bilingual skills (Spanish/English) are an asset.
• Experience in care coordination or community based social work preferred.
• Effective communication, documentation, organizational, and critical thinking skills.
• Ability to establish and sustain trusting, supportive relationships with individuals involved with violence.
• Ability to engage respectfully with individuals from varied backgrounds and life experiences.
• Flexibility to work evenings, weekends, and participate in 24/7 on-call rotation.
• Candidates with personal or professional experience navigating violence, trauma, incarceration, or the criminal justice system are well-suited for this role. Candidates must have a minimum of 2 years since release from incarceration, with no pending criminal cases or prior convictions for sexual assault, child abuse, or domestic violence.
• Willingness to complete required training
• Must possess a valid driver's license and maintain active automobile insurance, as the position requires transporting participants to appointments and services.

Physical Demands

• Ability to transport self, between Center For Family Services sites, community organizations, participants' homes as needed to support recovery and program goals.
• Ability to sit, stand, walk, or remain in a stationary position for extended periods of time (up to 4 hours) while conducting bedside support, documentation, meetings, or participant engagement.
• Ability to respond to participant crisis situations, during evenings, nights, and weekends as part of support to participants and the 24/7 on-call rotation.
• Ability to travel and represent the program at local, state, and national trainings, conferences, and stakeholder meetings to support professional development and alignment with best practices.
