Nature Of Work

A Social Worker or Case Manager is responsible for providing professional social services to adults and/or families in various program areas including mental health, chemical dependency, family facilitation, developmentally disabled/vulnerable adults, childcare/child and adult foster care, Child Welfare, Child Protection, and intake support services. Duties include assessment, formulating client treatment/service plans, case management, crisis intervention, safety planning, referral services, family group conferences, and other social service activities in accordance with program requirements, regulations, statutes, mandates, department guidelines, procedures, and data privacy requirements.

Communication
  • Internally – All HHS staff.
  • Externally – State/county/city entities and the public.
Supervision

None

Essential Work Functions
  1. Conduct comprehensive initial and ongoing assessment interviews to evaluate individuals/families’ backgrounds; develop case plans; make recommendations and referrals.
  2. Gather evidence and data for court documentation.
  3. Maintain regular contact with clients through various visits and coordinate with other agencies.
  4. Implement solutions and monitor client progress, preparing reports accordingly.
  5. Maintain detailed documentation and complete mandated forms within deadlines.
  6. Assess safety and risk, develop safety plans, and coordinate with relevant parties.
  7. Use a person-centered approach to determine service needs and referrals.
Other Work Functions
  • Perform related work as required.
  • Coordinate with departmental and community agencies.
  • Attend mandated trainings and continuing education.
Minimum Qualifications
  • Four-year degree in a Human Services related field.
  • One year of experience.
  • Ability to pass required background checks and testing.
Knowledge, Skills, and Abilities
  • Knowledge of social work principles, interviewing, and record-keeping.
  • Understanding of individual and group behavior, family systems, and social welfare research.
  • Ability to apply casework skills, work collaboratively, and manage time effectively.
  • Valid MN driver’s license and experience with Microsoft Office.
  • Effective communication skills and ability to establish working relationships.
  • Self-motivated, organized, detail-oriented, and team-oriented.
Physical Demands & Work Environment

Work primarily in an office environment with some travel and physical activity, including sitting, standing, walking, reaching, typing, and dealing with challenging behaviors or hazardous conditions.
