Job summary

Catholic Health Initiatives is seeking a dedicated Social Worker for Lasting Hope Recovery Center in Omaha, NE. You will assess psychosocial, environmental, financial, and discharge planning needs and develop individualized plans of care in collaboration with patients, families, physicians, nurses, and the interdisciplinary team to ensure safe, timely transitions.

Qualifications

  • CSW certification in Nebraska required.
  • Bachelor's degree in Social Work required.
  • Basic Life Support (BLS) certification required.

Responsibilities

  • Assess psychosocial, environmental, financial, and discharge planning needs to develop individualized plans of care.
  • Collaborate with the interdisciplinary care team to coordinate safe discharge plans and timely referrals.
  • Advocate for patients and families by connecting with support services and financial resources.
  • Educate patients, families, and healthcare team members on services and options for transitions of care.
  • Document assessments, interventions, and care coordination activities in keeping with standards.
