Northwest Texas Healthcare System in Amarillo, TX is seeking a Social Worker (BA/BSW) to provide social services and discharge management that support quality continuum of care.

Responsibilities include completing pre-discharge evaluations with patient/family interviews and the healthcare team, and developing viable intervention plans considering illness, living situation, support systems, and financial status.

Qualifications

• Bachelor's degree in social work, psychology, or related field.
• Minimum of two years of relevant work experience (e.g., customer service, case management, or discharge planning).
