Yanceyville Rehabilitation And Healthcare Center in Yanceyville, NC is hiring a Social Work Assistant for a full-time, 40-hour position in a long-term care setting. The role focuses on meeting residents' medical-related emotional and social needs in accordance with regulations, with a supportive management team and comprehensive benefits.

The successful candidate will interview residents, coordinate admissions and transfers, assist with care planning, and maintain resident dignity and privacy

Qualifications

• Enjoys interacting with the elderly and has good communication and problem solving skills.
• Experience at any level in a Long Term Care Facility.

Responsibilities

• Interview Residents to assist in development of social history and psychosocial assessment.
• Orient Residents and families to the facility and Residents' Rights.
• Assist with coordination, intake, admission and transfers within the facility.
• Coordinate inspection of Residents' belongings to ensure proper labeling and inventory.
• Address Residents' concerns and follow up in a timely manner.
• Provide 1-to-1 individual visits with Residents as needed.
• Assist Residents with activity programs of choice.
• Report hazards, damaged equipment and supply issues; follow safety precautions.
• Communicate with Residents, Visitors, families, peers and supervisors tactfully.
• Answer calls and transfer calls/paging as needed.

Summary/Objective

In keeping with our organization's goal of improving the lives of the Residents we serve, the Social Work Assistant ensures that the medically related emotional and social needs of the Resident are met and maintained on an individual basis and in accordance with current federal, state, and local regulations.

Essential Functions

• Interviews Residents to assist in the development of a comprehensive social history and psychosocial assessment that includes the Resident's problems, strengths and preferences, including cultural, religious and ethnic background and implications for the care plan.
• Orients Residents and families to the facility, its service limitations and Residents' Rights.
• Assist with the coordination, intake, admission and Resident transfer within the facility.
• Coordinates the inspection of Resident's belongings to ensure they are properly labeled and inventoried.
• Assists Residents with concerns, completing concern form and addressing the concern in a timely manner.
• Provides 1 to 1 individual visits with Residents when requested, on a regular basis.
• Assists Residents to Activity Programs of choice.
• Reports all hazardous conditions, damaged equipment and supply issues to appropriate persons. Follows established safety precautions when performing tasks and using supplies or equipment.
• Answers and responds to call lights promptly and courteously when working in Resident care areas.
• Maintains privacy, comfort and dignity of Residents and interacts with them in a manner that displays warmth, respect and promotes a caring environment.
• Communicates and interacts effectively and tactfully with Residents, Visitors, families, peers and supervisors.
• Assists with answering phones and transferring calls/paging when needed.

Required Education And Experience

Enjoys interacting with the elderly, good communication and problem solving skills, good computer skills.

Experience at any level in a Long Term Care Facility
