Job summary

The University of Rochester invites applications for a Social Work Assistant in the Psychiatry SMH Social Work unit located at 601 Elmwood Ave, Rochester, NY 14642. This full-time position involves collaborating with unit social workers to support patients and families during mental health treatment and discharge planning.

Responsibilities include coordinating discharge plans, outreach to patients after discharge, documenting interventions, and contributing to a culturally competent care

Qualifications

• Bachelor's degree in Social Work required
• Or equivalent combination of education and experience
• Clinical or Community Social Work experience in health setting preferred
• Prior experience working in acute psychiatric or behavioral health clinical setting preferred

Responsibilities

• Rounds on identified Adult inpatient psychiatry patients prior to discharge date/time for a warm handoff prior to their discharge from the unit for review of discharge plan via teach back method.
• Reaches out to discharged patients from Adult Inpatient Psychiatry, telephonically for two weeks post discharge to address any barriers identified in the discharge plan.
• Assists the Unit Social Worker in the coordination of the patient's identified discharge plan. This may include referrals, continuity of care, safety planning and/or assistance with the completion of other necessary discharge documentation.
• Documents interventions in compliance with Division, Department and Hospital Policy, to include any safety plans or other documentation crucial to discharge planning and continuity of care.
• Attends Social Work Division and Department Staff meetings, as required. Participates in activities of the Social Work Division including but not limited to timely completion of time reporting and other Hospital or Division forms; Hospital and Division mandatory in-service training programs and UHS reviews.
• Subscribes to the Departmental goal of creating a culturally competent environment by treating patients, families, trainees, and co-workers in a sensitive manner with appropriate attention to cultural differences. Participates in available cultural competence events and training appropriate to job duties.
• May provide social work assistant coverage and back-up as needed during the absence of co-workers in the inpatient services.
• Other duties, as requested and assigned.

Responsibilities:

The social work assistant will assist the unit social workers providing services to patients and their families admitted for mental health treatment. As a member of the interdisciplinary treatment team, the social work assistant participates in the development and implementation of the patient's treatment and discharge plan. The social work assistant is responsible for assisting the unit social worker with overall coordination of the discharge planning process for patients hospitalized on Adult Inpatient Psychiatry, and Child and Adolescent Inpatient Psychiatry. The social work assistant provides social work treatment in response to patient/family psychosocial problems which may impede care and/or discharge. The social work assistant develops interagency relationships and mobilizes community resources on behalf of the patients and/or their families. This social work assistant will provide follow up telephonic support to patients post discharge from the adult inpatient units for a period of 2 weeks to address barriers identified.

MINIMUM EDUCATION & EXPERIENCE

• Bachelor's degree in Social Work required
• Or equivalent combination of education and experience
