Job summary

Harmony Home Health and Hospice, LLC is seeking a Hospice Social Worker (MSW preferred, BSW acceptable) to support patients and families during serious illness and end-of-life care. This role offers Per Diem with full-time and part-time opportunities and travel up to 50%.

Responsibilities include conducting psychosocial assessments, collaborating with physicians and interdisciplinary teams, providing emotional and grief support, educating families on resources and advance care planning, and

Qualifications

• Bachelor’s or Master’s degree in Social Work (MSW preferred)
• 1 year healthcare or hospice social work experience
• Strong understanding of psychosocial aspects of illness and end-of-life care

About the Role

We’re seeking a Hospice Social Worker (MSW preferred, BSW acceptable) to support patients and families during serious illness and end‑of‑life care.

Key Responsibilities

• Conduct psychosocial assessments of patients and families.
• Collaborate with physicians and interdisciplinary teams to develop and implement individualized care plans.
• Provide emotional and grief support to patients and families.
• Educate families on community resources, financial assistance, and advance care planning.
• Help patients navigate practical concerns such as housing, insurance, and end‑of‑life planning.
• Maintain accurate documentation in compliance with state, federal, and organizational standards.
• Conduct follow‑up visits to assess evolving patient and family needs.

Qualifications

• Bachelor’s (BSW) or Master’s (MSW) degree in Social Work (MSW preferred).
• At least 1 year of healthcare or hospice social work experience.
• Strong understanding of psychosocial aspects of illness and end‑of‑life care.
• Valid driver’s license and reliable transportation (home visits required).
• Current CPR certification.
• Experience in mental health or crisis intervention is a plus.
