The Ohio State University seeks a Social Worker 2 at East Hospital to provide psychosocial assessments, counseling, crisis intervention, and discharge planning. You will help patients and families access community resources and coordinate with care teams to support recovery and safe transitions between care settings.

Requires a Master of Social Work and Ohio LSW licensure; full-time, first shift, on-site. A background check and potential drug screen may be required after offer.

Social work services are provided to patients/families to meet their medically related social and emotional needs as they impact on their medical condition, treatment, recovery and safe transition from one care environment to another.
