Job description

Position Overview

The Social Worker serves as a key member of the case management team, providing holistic, person-centered support to admitted claimants and their families. This role includes responsibilities such as resource navigation, advocacy, and managing complex cases. The Social Worker will use advanced de-escalation techniques, trauma-informed approaches, and problem-solving strategies. This position is Exempt under the Fair Labor Standards Act (FLSA).

Duties and Responsibilities

• Manage an assigned caseload by providing ongoing coordination, support, and proactive follow-up to participants and their families, ensuring needs are assessed regularly, services are aligned with Program guidelines, and communication is timely, accurate, and compassionate.
• Serve as a point of contact for escalated concerns, managing high-stress interactions with professionalism and empathy.
• Apply advanced de-escalation techniques and trauma-informed care principles and provide training to team members to effectively manage conflict and build trust.
• Identify root causes of claimant concerns and barriers to care.
• Provide counseling, crisis intervention, and resource navigation tailored to complex family dynamics.
• Address concerns raised by following up with families, clarifying program policies, and identifying actionable solutions.
• Connect claimants with appropriate community services, benefits programs, and support networks.
• Maintain accurate and confidential records in compliance with HIPAA and program policies.
• Participate in interdisciplinary team meetings to review cases and coordinate care strategies.
• Assist in developing workflows and protocols for integrating social work into the claimant support process.
• Participate in special projects and other duties as assigned.

• Must possess/maintain a valid driver’s license.

Education & Experience

• Master’s degree in Social Work (MSW)
• Eligibility for or possession of LCSW, LMSW, or equivalent Virginia licensure
• 5+ years of experience in social work, case management, or human services.
• Experience in high-intensity environments such as behavioral health, crisis response, family trauma, or grief.
• Experience working with children with complex medical needs, disabilities, or chronic conditions, or experience working with disabled populations and/or in medical/disability-related settings strongly preferred.
• Must maintain confidentiality and abide by HIPAA regulations.
