Job summary

Elevance Health is seeking a Health Coach/Clinical Social Worker contractor to guide members and program staff in addressing psychosocial barriers to health. Primary duties include outbound calls for health coaching and helping participants access resources to improve health management.

Requirements: MS in Social Work or related field with at least 3 years clinical experience; current LCSW license; healthcare/call center experience preferred. Job Type: Contract; Mon-Fri 8:00 AM–5:00 PM.

Provides consultation to staff members regarding methods and approaches to help participants recognize and overcome psychosocial barriers to better health.

Requirements

Requires MS in Social Work or related behavioral health field and minimum of 3 years of clinical experience; or any combination of education and experience, which would provide an equivalent background.

Current license as a Clinical Social Worker (LCSW) in applicable state(s) required.

Relevant call center and/or health care specific professional experience preferred.

Monday-Friday, 8:00AM-5:00PM

Proficient with Microsoft products. Prior experience with behavioral health case management a plus.

Elevance Health operates in a Hybrid Workforce Strategy. Unless specified as primarily virtual by the hiring manager, contractors are required to work at an Elevance Health location at least once per week, and potentially several times per week. Specific requirements and expectations for time onsite will be discussed as part of the hiring process. Candidates must be within a reasonable commute to a PulsePoint location.
