Job summary

St. John’s United Home Health & Hospice is seeking a Medical Social Worker (MSW) to provide psychosocial services for hospice and home health patients and their families, including a bereavement program.

The MSW will assess psychosocial status, plan interventions, maintain clinical records, collaborate with staff and outside agencies, and participate in discharge planning; travel to patients’ homes is required.

Qualifications

• MSW from an accredited school.
• CMS/Medicare regulatory requirements for home health and hospice.
• Minimum of two years' healthcare experience; home health/hospice preferred.
• Ability to work independently in patients' homes and office settings.

POSITION TITLE: HOME HEALTH/HOSPICE MEDICAL SOCIAL WORKER (MSW)

RESPONSIBLE TO: HOME HEALTH & HOSPICE ADMINISTRATOR

EXPOSURE LEVEL: II

EXEMPTION STATUS: EXEMPT

SUMMARY: St. John’s United Home Health & Hospice is a joint venture partnership between Billings Clinic, Big Sky Senior Services and St. John’s United. St. John’s has full responsibility for managing and growing this emerging home-based service agency. The MSW will serve in several developmental roles inclusive of providing professional medical social services for both hospice and home health. Additional responsibilities will include a bereavement program. It is anticipated that as the agency grows, and additional staff positions added, that the predominant work responsibility will be medical social services primarily to hospice and home health patients and their families.

ESSENTIAL JOB FUNCTION:

• 1. Provide living opportunities within nurturing environments of hope, dignity and love.
• 2. Assesses the psychosocial status of patients and families.
• 3. Carries out social evaluations and plans interventions or support services to patients and family/caregivers. Makes referrals as appropriate.
• 4. Assists with maintaining a bereavement program.

• 6. Maintains collaborative relationships with organizational personnel and outside agencies.
• 7. Participates in discharge planning as appropriate.

REQUIREMENTS:

• Minimum Requirements: Minimum of two years experience in health care, home health / hospice experience preferred.
• Education Requirement: Meet or exceed CMS/Medicare regulatory requirements for home health and hospice. MSW from accredited school.

PHYSICAL DEMANDS:

• Requires minimal lifting functions
• Requires moderate bending, standing and reaching activities

WORKING CONDITIONS:

• Works in client’s homes and in office areas. Driving and travel required.
