Springfield Rehabilitation & Healthcare Center is seeking a part-time Social Worker to support residents with psychosocial needs through assessment, care planning, and coordination of services. The role emphasizes counseling, collaboration with interdisciplinary teams, and ensuring quality of life for residents in a long-term care setting.

The ideal candidate holds at least a Bachelor's in Social Work, with preference for a Master’s degree and 1+ year healthcare social work experience.

Qualifications

• Bachelor's degree in Social Work required; MSW preferred.
• Minimum 1+ year healthcare social work experience.
• Experience in a nursing home or rehabilitation center preferred.
• Strong communication with residents, families, and staff.
• Knowledge of regulations governing nursing facilities.

Responsibilities

• Evaluate residents to assess psychosocial needs and develop care plans.
• Address resident goals and emotional needs; communicate with families and case managers.
• Maintain positive relationships with residents and families.

Daily Pay!

$25/hr - $30/hr (all inclusive)

Responsibilities for Social Worker:

• Evaluate current, new, and prospective residents to assess psychosocial needs, strengths, and limitations.
• Address resident concerns, goals, and emotional needs while maintaining ongoing communication with residents, families, and case managers.
• Build and maintain positive, supportive relationships with residents and their families.
• Coordinate care and collaborate with interdisciplinary teams, including physical therapy and other clinical departments.
• Develop, implement, and update individualized psychosocial care plans for residents.
• Participate in care planning meetings and support continuity of care throughout the resident’s stay.
• Provide counseling, emotional support, and resource referrals as needed.
• Ensure compliance with all applicable facility policies, regulations, and documentation standards.

Qualifications for Social Worker:

• Must be able to work full-time hours and maintain a flexible schedule.
• Bachelor’s degree required; Master’s degree strongly preferred.
• Minimum of 1+ years of experience as a Social Worker in a healthcare-related setting required; experience in a nursing home or rehabilitation center strongly preferred.
• Strong knowledge of regulations, policies, and guidelines governing nursing care facilities.
• Excellent communication and interpersonal skills with the ability to interact tactfully with residents, families, staff, and external agencies.
• Ability to work effectively in a fast-paced, team-oriented healthcare environment.
